Referee reports are generally not required for recreational games, but are needed in the following situations, per USSF:
- Sending-off offenses were committed, i.e., a red card was issued to a player or coach
- Serious injury was suffered by a participant
- Spectator issues
- Field issues
- Game delays
- Abuse or assault (verbal and/or physical) directed at any members of the referee team

Be sure to use language consistent with the Laws of the Game, and describe the offense in such a way that will be helpful to the Discipline Committee, which will use the SABR Discipline Chart.
